The St. John's - Independent Living Resource Centre is conducting its annual Consumer Satisfaction Survey. This survey is for persons who have utilized services at the ILRC over the past year.

Please use the link below to complete the survey online. When you are filling out the survey, if there are sections that don't apply to you please skip to the next section (program) that you have received services from. When you have answered all sections that apply to you, then go to the end of the survey and click on 'FINISH' to make sure your answers are added to the results.

The deadline to submit your completed survey is August 13 2010.
